The form only provides proof that the Audit was completed, no Financial information is requested. Please download, complete the form with participants signatures. Louisiana Ladies Member Auxiliary Insurance Renewal is now available online as well as a paper form. This year’s premium is $2.50 per member. Auxiliaries do not pay for Honorary members, but total member count need to be listed for the Proof of Insurance certificate. The current roster will be required with the premium renewal form.
Louisiana Ladies Auxiliary Association Voucher Form is required when requesting reimbursement or a payment to be made. The voucher can be downloaded. Vouchers along with receipts or invoices must be turned in to the Treasurer.
